Who asks for a football medical certificate?

In the UK there is no single rule covering all football. The requirement is set by whoever is running the competition or accepting the registration, which is why two players at similar levels can face completely different paperwork.

If you are registering with an English club, check whether the request comes from the club itself or from a county FA. County FAs generally rely on self-declaration, whereas individual clubs and academies set their own medical policies and can be stricter.

Cardiac screening and football

Football has a closer relationship with cardiac screening than most amateur sport, following well-publicised incidents in the professional game. The FA supports cardiac screening for players in the elite pathway, delivered through specialist programmes, and some academies expect a screening record alongside a certificate.

Screening and certification are separate things. A cardiac screen is a test, usually an ECG with an echocardiogram where indicated. A certificate is a clinical statement. If your club has asked for screening, you need the test itself, and an online certificate does not replace it. If your club has asked for a certificate and you already hold recent screening results, upload them so the reviewing doctor can take them into account.

Combat and contact sports

The same certificate format covers most other contact sports. Boxing, kickboxing, MMA and martial arts competitions commonly require doctor clearance confirming fitness to compete in contact events, and amateur boxing in particular is governed by its own medical rules through the relevant national association.

Where a combat sport is licensed, the licensing body's own medical requirements take precedence over any general certificate, and those often include in-person examination and eye testing. Check the requirement with the association before applying, because a general fitness certificate may not be accepted in place of a licence medical.

How to get a football medical certificate online

  1. Complete the online consultation form. Give your medical history, the club, trial or tournament involved, and any wording the organisation has specified. Upload their form if they publish one.
  2. A GMC-registered doctor reviews your submission. The reviewing doctor considers your history against the demands of competitive football, including any cardiac history or previous injury, and may request further information.
